I voluntarily allow my child to participate in the programs offered by Future Entertainers & Young Entrepreneurs Academy.

I certify that my child is physically able to participate in academy activities. I understand that all tumbling, movement, and beginner stunt activities are taught in a controlled and supervised environment focused on safety and performance training only.

I agree to release and hold harmless Future Entertainers & Young Entrepreneurs Academy, its instructors, staff, volunteers, affiliates, and venue partners from liability, claims, damages, injuries, or expenses arising from participation in academy activities.

I understand that parent supervision and participation are encouraged, especially for younger students.

I acknowledge that tuition payments are non-refundable unless otherwise approved by academy management.
